VOLUNTEER APPLICATION

A few items that are especially helpful to know at the start of the application process:

Coverage is only intended for physicians who:
- are retired from active practice,
- are providing direct patient care on a volunteer basis only if less than 10 hours per week.
- Only Occurence coverage with limits of $100,000/$300,000 will be provided.
- Please provide written documentation from the clinic confirming your volunteer status and hours.

You must be a member of the Texas Medical Association or have a membership application pending to obtain coverage through TMLT. To start this process, please visit the TMA web site www.texmed.org or call 1-800-880-1300

Premium Payment:

Premium is $1,200 annually and fully-earned.
Premium must be paid in full prior to binding coverage.
No payment plan is available for this coverage.
